DSA has created a Personal Health Record (PHR) Card, a secure smart card which stores a patient’s medical information. Cardholders keep the credit card-sized card with them at all times, allowing medical professionals to access the data on the card during routine visits and in times of emergency. PHR cards can be updated by the cardholders at home, and by their physicians. Information that may be stored on the card includes:
- Demographics
- Surgical History
- Primary Physician Information
- Insurance Information
- Emergency Point of Contacts
- Legal Guardian
- Living Will
- Medications
- Vaccinations
- Allergy Information.
